Our approach to medication & nutrients

Stabilizing and restoring your brain, rebuilding the body

Medication stabilizes your brain. Nutrients rebuild it. At NorthStar, both are guided by your genetics and managed by your care team. Your treatment is precise, often temporary, and designed to give your brain exactly what it needs to heal.

The role of medication

Medication is the bridge, not the destination

If you're nervous about medication, you're not alone. "Will I be on it forever?" "Am I just replacing one thing with another?" Those are fair questions, and they deserve honest answers.

The medications used at NorthStar don't get you high. They don't create euphoria or intoxication. They calm things down.

They quiet the cravings, steady your mood, help you sleep, and protect your ability to think clearly. Think of it like a cast on a broken bone. The cast doesn't do the healing, the bone does. Without it holding things steady, the bone can't set properly. That's what medication does for a brain that's been changed by chronic substance use.

Your brain has been altered by chronic substance use. Neurotransmitters, receptors, the regulation of mood and craving, all of it shifts under sustained alcohol or substance exposure. Medication relieves that chemistry so you can do the work of healing.

And it's not necessarily forever. The goal is always your health and independence, not a lifetime prescription.

Precision medicine: our 30+ medication options

NorthStar's founders spent a year reviewing over 2,600 peer-reviewed research papers1 to find every medication with real evidence behind it for alcohol and substance use treatment. Because of that, NorthStar has access to more than 30 medications dedicated to treating AUD and SUD, not just the temporary mental health medications some inpatient programs rely on. Pharmacogenomic testing tells us which of those medications will actually work for your biology, so we're not guessing.

How medications help

What different medications do in treatment

A focused driver

Quieting your cravings

Cravings aren't a willpower problem, they're your brain sending a chemical signal. Certain medications help restore balance in the reward pathways so those signals get quieter. When the noise dies down, you can actually make choices based on what you want, not what your brain is demanding.

Supporting your taper safely

Stopping alcohol suddenly can be life-threatening in some cases. NorthStar brings you down gradually while medications step in to support the same calming receptor system that alcohol was hitting. You don't feel high. You don't feel numb. You just feel steadier. And it's temporary for most as brain chemistry returns.

Restful sleep

Helping you with sleep

Even though alcohol and substance use may initially help you fall asleep, they destroy sleep quality. When the alcohol or substance is gone, your brain has to relearn how to sleep, and medication can help bridge that transition while your sleep architecture restores.

Treating what's underneath

Cravings aren't a moral failing or a sign you're weak. They're your brain sending a chemical signal that results in the craving, and that signal can get quieter. The right medication, matched to your biology, helps quiet your cravings and support your taper safely.

Medications in combination

Roughly 60-70% of patients living with AUD or SUD also live with a co-occurring mental health condition, depression, anxiety, PTSD, or others. Treating what's underneath isn't optional. It's foundational.

Improving outcomes longterm

Nationally, 78% patients who use medications as part of their treatment show significantly better long-term outcomes than those who don't.2 This isn't a NorthStar figure, it's the research consensus across decades of clinical studies. Medication-assisted care works.

A safer path forward

Why we taper instead of going "cold turkey"

A doctor would not tell you to stop a blood pressure medication or many antidepressants cold turkey. You would be weaned off gradually, with monitoring, so your body could adjust. Medication for AUD and SUD works the same way.

Most addiction treatment continues to either tell people to stop drinking, or removes them from all substances immediately when they go inpatient. NorthStar approaches this differently.

We taper patients off alcohol using a structured, medically supervised plan, bringing your intake down gradually at a pace your body can handle, while medications step in to support your nervous system through the transition.

This approach has been used in Europe for decades and was validated in an Australian study of 71,000 patients3 as safe and effective at scale. The difference patients feel is real. Less anxiety. Fewer withdrawal complications. Dramatically lower seizure risk. And a transition that feels manageable instead of terrifying.

For substances that are legally available - alcohol, prescribed benzodiazepines, prescribed stimulants like Adderall, THC - we taper carefully and clinically. We do not taper illicit substances. The taper is paced to your body's response, supported by medications that protect against withdrawal complications, and monitored throughout.

Nutrient repletion, putting back what was taken from you

Medication steadies the brain. Nutrients rebuild it. You need both, and neither works as well without the other.

Here's what happens over time with chronic alcohol or substance use: your body gets stripped of the vitamins, minerals, and amino acids your brain depends on to produce the chemicals that regulate your mood, your sleep, your anxiety, and your cravings. Alcohol destroys the gut bacteria that help you absorb nutrients. It damages the lining of your intestine. It flushes minerals like magnesium out through your kidneys every time you drink. And it replaces real food with empty calories.

The result? A brain that's running on fumes. It can't make enough serotonin, so you feel anxious and depressed. It can't make enough dopamine, so nothing feels good or worth doing. It can't regulate its own stress response, so everything feels like too much. Those aren't character flaws. They're nutritional deficiencies, and they're fixable.

Why your multivitamin did nothing

If you have ever taken a daily multivitamin and felt no different, you were probably right. Most supplements use the cheapest, most generic form of each nutrient and simply assume your body can absorb and convert it. For a lot of people, and especially for people whose bodies have been through years of heavy drinking, that assumption is wrong.

Folate is the clearest example. Standard folic acid has to be converted by your body before your brain can use it, and a large share of people carry a gene variant that makes that conversion slow or incomplete. Give them folic acid and most of it never arrives. Give them the already-converted form, 5-methylfolate, and the same nutrient finally reaches the brain that needs it. The nutrient was never the problem. The form was.

"Give the brain back what was stripped from it, and it starts to work again."

This is why we don't hand everyone the same bottle. We read your genetics and your lab results, choose the forms and doses your body can actually use, then keep checking that they are working. Done this way, repletion isn't a wellness add-on. It is how a depleted brain gets the raw materials it needs to heal.

What your brain & body needs

A look at the 20+ key nutrients we restore

B1, B6, B9, B12 & folate

These are what your brain needs to produce neurotransmitters. When they're depleted, you get anxiety, depression, brain fog, memory problems, and bone-deep fatigue. NorthStar uses genetically guided forms, like 5-methylfolate instead of standard folic acid for patients whose genetics show they can't convert it on their own, so the nutrients actually get where they need to go.

Magnesium

This is one of the most important and most depleted nutrients in people with AUD. It is responsible for more than 300 bodily processes, and it's commonly depleted in patients living with AUD. Restoring magnesium supports sleep, anxiety regulation, and muscle recovery. Addressing magnesium is often one of the first things that makes patients actually feel, calmer, steadier, more like themselves.

Amino acid precursors

These are the raw materials your brain uses to make serotonin (from tryptophan) and dopamine (from tyrosine). They come from protein, but protein has to be broken down through a healthy gut. When alcohol has destroyed your microbiome, that pipeline shuts down. NorthStar restores these building blocks directly while working to rebuild your gut health so your body can eventually do this on its own.

Iron, Zinc, vitamin D, and trace minerals

These round out the picture and act as co-factors. They support the enzymes that allow other nutrients to do their job. Iron in particular shows up throughout the methylation cycle, which is central to how your body produces neurotransmitters.

A difference maker in treatment

What this looks like in practice

25%
average improvement in cognitive function within the first month
40%+
cognitive improvement by the end of the year

NorthStar tracks cognitive function throughout your year of care. The numbers are consistent and they're striking: patients see an average 25% improvement in cognitive function within the first month. By the end of the year, that number climbs past 40%.

Medication calms the storm. Nutrients repair the structure. Together, they give your brain what it actually needs to heal, and the results show up not just in how you feel, but in what the data proves.
